Checklists
The DICES® System for Risk Assessment and Management gives you access to the following checklists to use with your clients, for the duation of your accreditation (currently 3 years, renewable). Each form is available to download and print, or complete electronically.
Part 1: DICES® Risk Assessment Checklists
- BRIEF-DICE
A guide to whether a person is likely to pose any significant risk to self or others.
- DICES-S
To help you make the judgement as to whether or not a person poses a significant risk of suicide.
- DICES-SN&V
To help you make the judgement as to whether or not a person poses a significant risk of self neglect, or is significantly vulnerable in any way.
- Substance-DICE
Helps you make a judgement about the risks associated with substance misuse, both to the subject and to those around them.
- DICES-V
To help you make the judgement as to whether or not a person poses a significant risk of violence to others.
- DICES-SA
To help you make the judgement as to whether or not a person poses a significant risk of sexually assaulting others.
- C&A-DICE
A checklist of the major risks that children and adolescents are susceptible to.
- Older-DICE
Evaluate risks that older adults are especially susceptible to.

DICES® Training.
Training to use the DICES® system is availabale in various formats, including a 2-day course (suitable for most people) and a 3-day course (covering everything in more depth with more discussion and examples, and is appropriate for ‘DICES® champions’ and senior staff who may wish to have that extra depth).
